Hospice & Palliative Care
Trio Healthcare facilities collaborate with hospice and palliative care service providers to offer compassionate care and support for their long-term care residents facing serious or life-limiting illnesses. Individuals who are already receiving hospice or palliative care when they become residents of one of our long-term care communities can continue to receive those services from their provider.
These specialized services address the physical, emotional and spiritual needs of residents and their families. Hospice and palliative care teams work alongside other caregivers and healthcare providers to ensure residents receive thoughtful, personalized care in a familiar and supportive environment.
Hospice and palliative care services can include:
- Pain and symptom management to improve comfort and quality of life
- Personalized care plans tailored to each individual’s medical needs and personal preferences
- Coordination with physicians, nurses and care teams to ensure seamless care
- Emotional and psychological support for individuals and their families
